Abstract

Calcified lower lumbar disc herniations are not common. A 43-year-old male patient presented with pain in the lower back and both legs. He also had signs and symptoms of impending cauda equine syndrome. Imaging studies revealed a huge calcified L4-5 disc herniation and marked neural compression. The patient was treated successfully by an anterior approach and fusion. The anterior approach to remove a huge central calcified lumbar disc herniation is a safe method of access when neural elements are severely compromised.
